specialized medical services.
It is always a good idea to read
your insurance member’s manual
or contact your insurance company
to find out what services are
covered and how to obtain them.
If you may self-refer, simply

to schedule an appointment.
If you must be referred by your
primary care physician, complete
the Self-Evaluation
Checklist and take
it to your doctor's appointment.
You and your primary care physician
can then decide the best course
of action for you. If you require
more specialized care, ask your
physician to refer you to Mid
Coast Swallowing and Heartburn
Center.
